Human Factors Associates, Inc. is pleased to announce that the company is considering upgrades to the Organizational Safety Effectiveness Survey (OSES). This online survey is currently used to assess safety climate, culture, and safety management systems effectiveness across safety critical industries, including Aerospace, Aviation, Hospitals, and Electric Utility, and nuclear facilities.
Updates to the survey will include new survey items regarding employee well-being, work – personal time balance, and adaptation to changes in national and global business operating conditions. Of course, we are anticipating incorporation of some artificial intelligence to improve. This far we have completed the functional architecture for an advanced AI – Agentic OSES software platform and are working toward development of a prototype for initial testing and promotion. While waiting for further news on completion, we are presenting some key events in our corporate history, below.

2018
August 2018 Completed the Organizational Safety Effectiveness Survey (OSES) Final Report for Airnorth, an Australian commuter airline, a subsidiary to the Bristow Group.
July 2018 Completed Final Report for NASA OSES Aircraft Division, Flight Centers climate – culture study.
February 2018 Completed Analysis and Final Report for Bristow Group OSES, Oil and Gas and Search and Rescue global air transport operation.
2017
May 2017 – NASA renewed HFA Contract for Organizational Safety Effectiveness Survey (OSES) for assessing safety climate and Safety Management Systems (SMS). This event marked the 7th year in support of NASA Aviation Safety.
May 2017 HFA is pleased to welcome the Bristow Group as a returning client for use of the Organizational Safety Effectiveness Survey – OSES.

June 2016 – Human Factors Associates completed Interviews and update to Organizational Safety Effectiveness Survey (OSES) for REACH Air Medical Services.

February 2016 – Dr. Ciavarelli served as invited speaker at the REACH Air Medical Services “Core Values Safety Seminar.” Held in Palms Springs.
January 2016 – Human Factors Associates finished the final report for the NASA OSES Culture Study. Seven NASA Flight Research Centers participated in the employee interviews and survey process.
2015
December 2015 – Human Factors Associates signed a two – year contract with REACH Air Medical Services for assessing safety climate – culture and safety management effectiveness.

November 2013 – Dr. Ciavarelli presented Applying HRO Survey Assessment Methods in a Culture Change Process, at the European HRO Conference held in Aix Provence.
November 2013 – HFA associates, Mr. Vittorios Coletta and Mr. Michele Pilia received commendation letter from the Italian Air Force (ITAF)Safety Directorate for analysis of ITAF OSES.
October 2013 – HFA completed the NASA OSES for assessing safety climate and safety management system status for NASA flight centers.
September 2013 – HFA completed the Italian Air Force (ITAF) OSES for assessing ITAF safety climate and culture.
May – October 2013 – Dr. Ciavarelli served as human factors expert on a National Academy of Sciences committee for selection of the best available and safest technologies for offshore oil and gas industry.
April 2013 – held an advanced workshop, Applying HRO Survey Metrics in a Culture Change Process, at the 6th High – Reliability Conference.
2012
October 2012 – HFA completed the Organizational Safety Effectiveness Survey (OSES) for the Allegheny General Hospital – Life Flight Air Medical transport bases.
October 2012 – Dr. Ciavarelli served an invited plenary speaker – Assessing Organizational Effectiveness as an Integral part a Safety Management System – Helicopter Advisory Committee Conference held in Houston, October 3, 2012.

March 2012 – HFA competed the final analysis and reports for the NASA Flight Centers Organizational Safety Effectiveness Survey (OSES). This work is accomplished in Collaboration with EX-3, one of HFA’s business partners.
2011
March 2011 – Dr. Ciavarelli served as invited speaker at the Annual Aviation Human Factors and SMS Seminar Real – World Flight Operations – Research Progress, Dallas.
February 2011 – HFA completed the final analysis and feedback phase of the Qantas Group safety Organizational Safety Effectiveness Survey (OSES). Analysis included comparison of safety climate normative results across aviation and aerospace.
January 2011 – HFA completed the Organizational Safety Effectiveness Survey (OSES) for all USA, Signature Flight Support facilities, and reported survey findings.
December 2010 – HFA completed a culture study report for Asia Pacific Economic Cooperation (APEC) entitled Seminar on the necessity of culture change to promote reporting safety issues: Case Study of Three Economies, Singapore, Peru and USA – in support of ICAO Safety Management implementation.
October 2010 – Dr. Ciavarelli lead a Culture Change Seminar sponsored by the Asia Pacific Cooperation. The Seminar was held in Lima Peru. Representatives from eleven Civil Aviation Authorities attended, including – USA, Singapore, Malaysia, Thailand, Philippines, Mexico, Chile, and Peru.

December 2009 – HFA completed analysis of the Organizational Safety Effectiveness Survey (OSES) for NASA Flight Centers, including Johnson and Houston Space Centers, and the Flight Centers at Dryden, Glenn, Langley, NASA Ames, and Wallop.
September 2009 – Dr. Ciavarelli was an invited keynote speaker at the Jorge Chaves Anniversary Aviation Safety Conference that took place in Lima Peru.
December 2009 – HFA completed application and analysis of the Organizational Safety Effectiveness Survey (OSES), for Qantas Airways.

2003- Human Factors Associates, Inc., was founded in October 2003, by Dr. Anthony Ciavarelli. HFA developed and applied the first commercial aviation safety climate survey for Embry – Riddle Aeronautical University.
